Choose the Claim Form (General Liability, Auto Liability and/or Collision, or Property) that coincides with the type of claim or incident you wish to file.  Please complete your claim form according to its specific instructions based upon the information available to you. The Property Claim Form will also require the completion of a Proof of Loss-Sworn Statement Form . We strongly encourage you to supplement your form with any additional information you believe is of value to the investigation of the claim.  If you are unable to attach the supplemental information at this time, please fax or mail the information to The Trust as soon as possible with a copy of your claim submittal. Workers' Compensation and Public Safety IOD Claims Forms
